Request Medical Records

To initiate a request for medical records from Vail Health Behavioral Health services simply download, print, complete and sign the following form:

Authorization to Release Protected Health Information (pdf)
Autorización para divulgar información médica protegida (pdf)
 

Instructions: Please complete ALL portions of this authorization and bring a printed copy with you to Medical Records at the address below. To expedite the process, you may email a scanned copy along with a copy of the patient's valid photo ID to medical.records@vailhealth.org OR fax the completed form and a copy of the patient's valid photo ID to Medical Records at (970) 470-6641. Please specify if you would like to pick up the copies in person or have us mail them to you. Unfortunately, we do not fax copies of patient medical records.

Note: Requests for medical records are processed in the order they are received. Medical records will be delivered within 10 to 14 days upon receipt of request.

Drive to the back of the Edwards Pavilion. Walk in through the door to the left of "Shipping & Receiving." Walk or take an elevator to the top floor. Medical Records is on your left as you step out of the elevator or off the stairs.
